Dear PHS Families,
We are fortunate enough to be able to provide PHS students with the opportunity for dental care from the services provided by Dental Safari on Tuesday, October 13th. They will provide an exam, cleaning, Fluoride, and sealants right here at the High School during school hours. This might be useful for students who have difficulty scheduling dental appointments, have trouble finding dentists who take Medicaid, don’t have a regular dentist, or any reason whatsoever.
You’ll find all the details of this program in the attached flyer. You can fill out the form here: http://www.dentalsafariforms.com/ which goes directly to Dental Safari. Another option is to fill out the attached form and return it by email or paper to the school nurse, Glenda Coffelt by October 6th. Dental Safari offers services, at no cost to the family, for students covered under Medicaid or Hoosier Healthwise. For students who have private dental insurance, they may receive Dental Safari services, most often with no co-pay. In addition, students who receive free or reduced services, but are not covered by either Medicaid or Hoosier Healthwise, may receive a grant for Dental Safari, bringing their cost to zero.
